Proof of residency must be uploaded before equipment pickup
Examples: utility bill, bank statement, etc.
Must show:
Current address
Dated within the last 60 days

To upload proof of residency to an account:

1. go to westlakeyouthfootball.com

2. login to your account

3. click on "my account" in the upper left corner

4. click on "members"

5. select the pencil icon next to the player's name

6. scroll down to Proof of Residency and click "select"

7. upload the photo of your form

8. click "save" - this MUST be done or the form will not save in your player's account.
