I understand that the submission of this application does not ensure an interview or a coaching position until official approval by the WYFL Board. If selected for a coaching position I am declaring that I will adhere to all standards and expectations set forth by Westlake Youth Football League.

I hereby release and agree to hold harmless from liability the Westlake Youth Football League and any of its officers and volunteers thereof, or any other person that may provide such information. The information I have provided is true and complete to the best of my knowledge.

If selected to be a coach, I understand that I will be subject to a thorough background check at my own expense and upon completion and passing of the background check will a final offer to coach be made
